Helping Patients Take Control

“The Chronic Care Management Program at Hope Clinic has helped me control my blood sugar not just with medications, but with the foods, exercise, and mental work that goes into it, too,” said Randy Mullinax. “Before, I was stuck, not knowing what kind of food I need, or good exercises for my neuropathy.”

Before starting the program, his blood sugar level was dangerously high, but now hes keeping it at a safe level. “I am learning so many things about coping,” he said, such as how to read nutrition labels to figure out which foods to avoid.

Hope Clinic started the chronic care management classes in March to provide patients with resources to manage their medical conditions, improve their overall wellness, and limit the need for emergency care, said Rina Yoder, BSN-RN. Weekly classes cover everything from nutritional guidance to mental health.

At group sessions, patients share their experiences and “we learn from each other,” Mullinax said.

Participants also meet individually with Yoder, who goes over lab results with them, answers questions, provides relevant handouts, and makes sure they are sticking to their goals.

Mullinax, who has several chronic conditions, reviewed his blood test results with nurse Yoder, and she explained how one issue affects another. Those one-on-one meetings made a big difference in managing his health, he said. “My wife and daughter tell me I look different and act different. It has really changed my life.”

The Hope Clinic provides quality medical care to people in Gwinnett County with limited access to health care, regardless of their ability to pay. Since 2002, the clinic has donated more than $8,500,000 in care.
